Six Flags Trip — Parental Permission & Liability Waiver

For purposes of this agreement, Metropolitan Baptist Church is hereinafter referred to as “the Church.”

By checking the box below, I confirm that I am the parent or legal guardian of the child registered for this trip and give permission for my child to participate in the Six Flags trip organized by the Church, including transportation to and from the event and participation in activities at Six Flags.

I understand that participation in this trip involves inherent risks, including but not limited to transportation accidents, falls, collisions, injuries, illness, extreme weather, amusement park rides and attractions, interactions with other participants or members of the public, food-related illness or allergic reactions, communicable diseases, and loss or damage to personal property.

I understand that these risks cannot be completely eliminated and that the Church cannot guarantee my child's safety. I voluntarily assume these risks on behalf of myself and my child.

To the fullest extent permitted by law, I release and hold harmless the Church and its pastors, employees, officers, directors, volunteers, adult leaders, agents, and representatives from claims, demands, damages, injuries, illnesses, losses, or expenses arising out of or related to my child's participation in the trip, including claims arising from ordinary negligence, except to the extent such a release is prohibited by law.

I understand that Six Flags and other third-party facilities and service providers maintain their own rules, requirements, warnings, and safety procedures, and I agree that my child must comply with those rules and with the instructions of Six Flags personnel and Church leaders.

I authorize the Church and its designated adult leaders, in the event of an illness or injury and when I cannot be reached promptly, to obtain reasonable emergency medical care for my child, including transportation to a medical facility when reasonably necessary. I understand that I am responsible for any medical, transportation, or other expenses incurred on behalf of my child.

I agree to inform the Church of any medical condition, allergy, medication, dietary restriction, or other circumstance that may be relevant to my child's safety during the trip.

I understand that participation is voluntary and that by checking the box below, I am agreeing to the terms of this Parental Permission & Liability Waiver.
